You certainly don't get a free pass at declaring to be whatever the hell you want. If he truly thinks he's 6 year old girl, he is mentally unstable. I'm not advocating at putting him to an asylum and take his freedom with it - more like giving him the help he needs, and a proper home (if such condition can be treated by/while living with his parents/relatives).

It costs money and resources to offer help for mental health related issues. Therefore it's easier for those in charge to slyly encourage people to brand each other 'bigots' for not bending over backwards to accept every little claim made by people who are clearly in need of major aid and support. When you think about how much money will be made after interviewing people like this and setting up 'charities' in their honour after calling them heroes it all begins to make more sense.
