    One of the problems with it is that it is not available at most pharmacies.

    Fairly regularly. Professional athletes often get pay raises like that. CEOs with stock options often do as well, although that's less "salary" and more monetary value of the compensation package.

    Nope most people are happy to get a raise around 1-2%, barring a promotion of course.
