The original Well of Eternity was what was left after the Pantheon had the keepers contain a leaking wound in Azeroth.

This is accurate. The original wound occured when Yshaarj was ripped from the ground, leaving a massive wound which was likely similar to the new one in Silithus. In order to contain the wound and let it "heal" they created the Well of Eternity from it as sort of a scab, so to speak.
