I normally do it with my eyes closed lying down. Sometimes even with a blanket or something like that on. And when I get really hardcore i turn on a fan. Pretty hardcore, I know.

Very well and usually on my sides, with my .357 Mag revolver under the edge of the pillow. Firm, cheap mattress. No problem sleeping, so no need for expensive pillows or mattress.
