Noobadin that's a pretty normal overclock for a Phenom II x4 and I have not seen too many that went too much farther.
Did you get a screenshot of after the OCCT test finished?
4ghz is a soft barrier for Phenoms x4's in the same way 5ghz is for i5 2500K's. Some chips can do it, some need absurd amounts of vcore, some just can't.
58c is getting pretty close to the 60-65c safe limit for Phenoms right? Or am I thinking of some other AMD chip?

Well, currently running some Prime95 27.7 testing on my 2500K@4.5GHz, 1.37v.
So far for my own personal tests from an old guide I had for the P8P67 boards, running a 20 minute test of 1344 FFT in 1 minute intervals and then a 20 minute test of 1792 FFT in 1 minute intervals have passed. Next up I'm going to play some Rift (I need it right about now with personal events that have happened today) and hopefully no crash will happen. If no crash happens and all is well, I'll either pull up a P95 blend test or do some good ol' Folding and take a nap/go to sleep. (Very tired with everything going on.)
Then again, I may just try and run a couple more 1344/1792 tests at 4.6GHz with about 1.39v and see what happens. :P Might need 1.4v.... I suppose I could see how much lower I could get 4.5GHz stable at though, but tbf, 4.4GHz already needs 1.345v. =/

Sigh. I remember now why I gave up and settled for 4.4GHz last August, BSOD error 124, one that is very difficult to figure out as it has so many possibilities as to what it is. idk if I want to bother.
