Majora's Mask - For the atmosphere, symbolism, narrative, characters and song of healing.
Banjo Kazooie - For the leitmotifs, world, characters, humour and atmoshpere.
Psychonauts - For the atmoshpere, world, characters, symbolism and humour.
Metroid Prime - For the atmosphere, scan visor, music and "narrative".
Eternal Darkness: Sanity's Requiem - For the narrative, atmosphere, sanity events and the uniqueness of the three gods' stories/monsters/spells.
That was hard.
No way I could narrow it down to just 5 ever, but top 5 of each platform, in order (clumping series or by the same devs):

My top 1 game of all time is Shadow of The Colossus. Never has a game moved me so deeply. I wish I could just forget about it and play it again like the first time.
After that, it's mostly dependant to a period of time.
