How do you avoid players

I have ignored/reported this guy multiple times. I have waited between queues to ensure we don't queue together.

But day after day I keep getting paired with this ass hat, and he's the rudest most negative "all about me" player I have ever met.

Trash talks to both sides (he's not even any good).

Also the ignore works for chat during score recap, and champion select... but when it comes game time... the ignore doesn't work.
