    NFL Blitz.. gods the arcade (yes, an actual coin-op arcade:P) ate so many of my quarters playing NFL Blitz, NFL Blitz 2000 and the like...

    Either that or Teenage Mutant Ninja Turtles: Tournament Fighters (which was.. SNES I think).. fricken awesome game...
    OR... Teenage Mutant Ninja Turtles: Manhattan Project (that 8-bit game devoured my coins at the arcade AND my time at home on NES)

    Vanilla EverQuest, didn't even have to think about it.

    Honorable mention for Chrono Trigger. (I think there was a fanmade attempt at this at one point, and square was like NOPE!)
