Act 2. First group of mobs. Lacuni and Wasps. I hate thess enemies since then. xD
Act 4 in a random dungeon wanting to grab loot did not notice a big ice ball forming on top of it, not to mention I barely survived the fight

Somewhere in Act 2. Died to a bunch of those damn sand wasps because someone talked to me and I turned around for a second.
Act II, the desert part with Zoltun Kulle, my first encounter with lunatics...
Act 3 against those plague dudes. I didn't just die once against them. Happened over and over and over. On normal. I was disappoint.

1st 4-5 deaths where due to massive lag spikes. the ones after due to misspulls
First time I died was in that castle where you have to help the soldiers raise the catapults or something (Act3 ??). Anyways, at some point I was asked to clear some trash mobs inside the castle and I got killed by those fat ass mobs that explode as soon as they touch you. Got 1-shotted 3-4 times, so annoying...
A friend's lvl 10 Barbarian died with the poisonous trees in act 1, i made it by a mere handful of HPs, i got a lvl 10 monk, not dead yet
First Act, those trees that spit out flowers that release poison.
I killed one, and it dropped a load of gold in the poison and I thought "it'll be fine, there's health globes in there too and it wont do much damage". Ran into the poison and died much faster than I thought I would
Act 1 Tree poop. I figured it wouldn't hurt that much and went in to collect the loots. D-X
My First death were in Act 1 with trees and poison. Died because i thought its damage shouldnt be that much Nab in gear
Coming into this thread I was certain Belial would be something like 60-70% of the answers. Belial 'phase 2' was just so much more challenging than anything in the game until then.
So, yeah, Belial. Over-complicated approach when I tried to vault out of the green crap to minimize downtime. Turns out that if you exhaust your discipline vaulting and then find yourself standing in a corner, you're screwed.
Server lag against a mortar mini-boss.

Act 2 Timed Cave. Didn't know what would happen if time ran out. (I expected it to kill me). So when time got low, I ran around trying to find the exit. I found a lot of mobs and got pinned. Turns out you don't die when time runs out.
