I'd prolly have to say any city in the Fallout universe. You can't even go for a stroll without being attacked by robots, giant scorpion, or mutants with laser gatling guns. To top that of everything is just radioactive :P

When it comes up for Great Cthulhu's awakening, the only plausible "downside" is that you, as a loyal follower, may be allowed to be among the first to sate His unending appetite. Innsmouth? I hear those people get to live under the sea for all of eternity. Immortality is bad?

Personally, I think the worst city to live in would be Dunwich when that unspeakable Thing went on a rampage.
