Firelord and Dragonslayer were two of the hardest titles to get while Sinestra and Ragnaros were current content in 4.0 and 4.2! There's no hard PvE title at this moment. Blame the nerfs!

Right now I think it would be the Insane.
"Salty", maybe? Or "Insane". Well - those are grindable I guess. A "Gladiator" or "Realm first anything" otherwise maybe?

Well apart from the impossible ones, R1 glad for sure, rest are just grinding.
R1 gladiator for sure, but even so, a lot of wintraders nowdays.
For the hardest, I would say the higher PvP titles (especially as there's just one week left) and then Herald of the Titans, even more so as you have to find 9 other players for that.
Cataclysmic Gladiator or any other rank one gladiator title meaning they are ranked first in the entire battlegroup 10+ servers!! Is so impressive it takes many hours of practice assuming you got it legitimately in 3v3 (wish I could get that title)
For those people that only pve a rank one gladiator plays very close to flawlessly in arena almost never gets locked out and knows positioning very well, while being able to play defensive and keep pressure up.
Some notable R1 players are Talbadar, Khuna, Braindeadly, and Snutz
