How do conspiracy theorists come up with stuff?

Normal thought process:

A, B, and C happen. X, Y and Z also happen. X, Y, and Z may be tangentially connected to A, B, and C, but are really their own little thing.

Conspiracy thought process:

A, B, and C happen, which causes Q, which causes H, which causes YY Alpha Mu, which leads directly to X, Y, and Z!

Imagination.

Blaming something on the government? Easy enough. People already know the military has bizarre plans, that the CIA does clandestine things that never see the light of day and citizens already have a severe distrust in their own government. A conspiracy theorist just works with all three factors, then doctors the evidence to create his theory.

Like claiming "there were no aircraft parts found in the rubble at the Pentagon" which is totally bogus, but idiots fall for that sort of shit if you ONLY show pictures of rubble without airplane parts.

Someone sees a hole in the official story. They see evidence that supports that hole. Then confirmation bias does the rest.
