As the spell's tooltip states, Revenge "Instantly attack an enemy and two additional enemies." In practice however, I am only able to hit my primary target unless the secondary target is virtually on top of the first. Anyone else having similar problems? Is this a known issue?

Well known. The "cleave" range on revenge is practically zero.

Range might be bugged, GC thinks its 5 or 8 yards. Its more like 2 yards in my game

Its probably 2 yard range like the fail slam on the ptr.

The only problem I have with it is that they took it from Arms Warriors and made it the abomination it is today. Definitely my favourite ability in Wrath of the Lich King for PvP.

They know there's potentially an issue with it, it'll likely be fixed/improved.
