If I'm reading it correctly though, it looks like it may only take two people reporting.

The OP was first reported on the 24th, and it slid by because it was just one. He was reported again, on the 27th, and within hours got the notice.

Recently a blue poster confirmed that they don't act on just one report of inappropriate behavior, that it takes multiple.
