Where did the second Moon go?

It's in the sky where it have always been, at least since it was re-inserted in MoP.
It just follows a pattern so it don't appear at a specific time but more random, it might even appear during the daytime.

It was removed in the expansions before MoP?

Pretty sure it's always been there, just not used in a whole lot of skyboxes. I remember the last time I saw it was during Cataclysm at night while leveling an alt through the Searing Gorge. It was one of the rare times both moons appeared in the skybox at the same time and they were on opposite sides of the zone. Usually one ends up going far out of the visual range of the skybox when the other appears, making it looks like one moon even though you are seeing both, just at different times from eachother.
