I'm using similar pattern for all classes with little differences here and there. Helps a lot when switching characters oftenly.
1,2,3,4 - main rotation spells (5 or higher is simply too far for me to press without breaking contact with other buttons, little hands :P)
Q - reactive/proc spells
E - utility (charge as example)
R - short CD interrupt
F - WTF defensive cooldown (IceBlock, Bubble or merged cooldowns via macro)
C - ability with short/mid CD (movement)
V - ability with short/mid CD (defensive)
Shit+Q - Burn macro (DPS, any spec)
Shift+E - quite random, usually DPS oriented minor ability
Ctrl+C - ethier class/race specific snare/stun removal or longer CD defensive ability
Ctrl+V - PvP Trinket (always, in PvE its empty)
Shit+C and Shift+V are various defensive cooldowns by class
Ctrl + 1,2,3,4 - two potions.. healing spells (for hybrids) or wierd situational spells for others
Shift + 1,2,3,4 - Any pure AoE oriented spells included in AoE rotation
F1 - Healthstone / HP Potion macro
F2 - self healing ability with CD
F3 - raid wide ability if available
F4 - not used...
Alt + 1,2,3,4 = as many as needed, long term buffs (party wide or self only)
Any other Ctrl+Shift combinations are random and used only if necessary. I do not use Alt modified for anything beside the long buffs. I always prefer shift
Also never understood "S" hate.. sure I don't use it in PvP, there is no reason. However there is also little reason to not have option to directly and at slower pace go backwards in PvE (tanking as prime example).
