Shift + 4 = Bear
Shift + 5 = Aquatic

Not the most effective binds but they are (quite literally) where I've always had them bound.

f5 - walrus, everything else the same.

Been that way since TBC, I use T for reply to/w, I just got so used to using R as flightform that it simply can't change now ^^

F keys. Not the most convenient but I'm so used to it by now. I bind pretty much everything else including main rotation to my mouse, so I have a lot of free left hand fingers anyway.

Mwheelup and down = Cat form
ShiftMwheelup= Travel form
CtrlMwheelup and down = Bear form
Shiftmwheeldown= Aquatic form.

Control + space = flying form - My standard flying mount button across all my characters

but when i try to bind Alt F4 to flight form my game crashes
