From what I remember you unlocked it when you completed the game.. Which would mean completing act 5. But might be wrong.

Nobody is sure atm.. What I mean is that they haven't commented on it and we haven't even asked. But one could speculate that if you did get a guaranteed legendary from first time Malthael.. then going there on freshly dinged 70 would be the best option to jump start your gearing process.

That's only for Diablo, not any other act bosses. You don't get guaranteed loot at max level.

You get a guaranteed legendary from Diablo *until* 61, but you also get a guaranteed legendary from Malthael. Also, yes, you unlock adventure mode by beating Act V, but you unlock it for *all* your characters. A lot of misinformation floating around this thread.

First time kills:
1-59 - Guaranteed legendary from act final boss.
60 - Guaranteed legendary from Diablo.
61+ - Guaranteed legendary from Malthael.

Adventure mode req:
Finish ACT V on one character.
