NVIC isn't at the top of my reliable source list. I can't take any source seriously that recommends homeopathy, chiropractic, and acupuncture as reasonable healthcare measures in their flu season PDF, and that's on top of all their other anti-vax stuff. Anyway, the CDC has a nice little tool to browse laws and such by state, too.

I can't say I'm familiar with any state that doesn't require at least some vaccinations. As far as waivers go, while they exist, I question how often they're really used.

Better than 4% in some states. About 0.4% of those can be attributed to medical reasons, leaving "religious" and "philosophical" waivers the remaining 3.6%.
