What addon is this??

So my character, sometimes randomly in combat, often at low hp % (though not always) i get spammed, literally spammed as fuck, by the sound/noise
"you will die" (i belive its the sound voice file from c'thun)

This is incredibly annoying, i can't figure out whats causing this ?? Someone help pls.
