Does the rage gain from the defensive stance have an internal cooldown and if yes is it a general cooldown or per player?
It is a cooldown, like 1-2 seconds or something. It is general and not per player. If 10 players hit exactly same time he would gain same amount of rage, but normally people who try to slip in dots will hit unevenly causing him to gain tons of rage.
Taken from one of the dungeon journals:
Defensive Stance – A defensive stance that reduces damage taken by 10% and grants 2 Rage when struck. Rage gain cannot occur more than once every one second.Attacks by players affected by Sundering Blow do not cause Nazgrim to generate Rage.

yes it seems to have a cooldown regardless of how many hit him for 1 second
for normal and below, nazgrim getting hit in defensive stance usually doesnt generate much rage anyway. its far worse having him in berserker with a few flags up cause then he gets like 20 rage per second due to the war banner effects.
Yes it does have a CD, you either want everyone to keep their hands off of him or have everyone attack him. If you're gonna have 1 guy hitting him and feeding him rage, might as well have everyone hit him so you at least get some dmg done on him in return.
