Beast Wars PS1 and/or the obligatory Superman 64. Neither were so bad it's good. They were just awful.

Seconding Superman 64. I rented that game back when Blockbuster was still a thing and it was bad enough that I still remember it today.

I've always been fairly selective when it comes to picking which games to play. Most "bad" games I play usually just end up not being my cup of tea.
However, I've seen A LOT of bad games. I think the worst games are those slopped together Simulation Games that lost their vision halfway through the creation process (and then were told without finishing the 2nd half). But I might have to second Ride to Hell: Retribution as the worst completed game I've seen in a good long while.

I don't usually buy games on a whim or pre-order as I like to check out the gameplay/story of most games before I play them. Having said that though, I got burned by Dungeon Siege 3... Given DS1&2 were both fairly good crpgs I grabbed the 3rd without my usual cautious approach and all it managed to do was remind why I was like the way I am in the first place.
Cataclysm.
