How to get as much follower gear as possible

What level profile should active followers have in order to maximise follower gear earned from missions and salvage?

Should the active followers have the highest possible ilvl?
Should you keep 1, or 3, at every level/ilvl range, and max the rest, or just spread them evenly across the 95-100, 615, 630 and 645 brackets?

The follower game got me by the balls, I have realised a few weeks ago that I'm going to aquire all the followers I can and level them all to max. Making followers 100 epic is straightforward, gearing them, however is going to be a much slower process.

Assuming that you have salvage / war mill / barracks, I guess I'd say that you want to reserve the blackrock upgrades for followers who only have level 600 armor or weapons, so you get the full 30 points out of that upgrade. After that, it's just a matter of chipping away at it I suppose.
