Does Vivi from FF9 count? While he's alive during the game, and he doesn't die during the actual game, he's dead at the point of the ending cinematic.

(Note: the original japanese version makes it clear that Vivi narrates the ending, and that his time has been used up some point before the ending happens)

Serves him right for killing the mercenaries who risks their life to help him in Northrend. Killing innocents people in Stratholme without hesitation..yes they were infected but the way he immediately did the slaughter without any hesitation is a big NO from me.
