We raid 4 nights, 4 hours each night. We take a 10 minute break every raid night half way through the night. 90% of the time it ends up being a 15m+ break though.

If you are talking about "Health" then realistically you should be taking a break every hour.

Where the computer work is intensive, good work station design should be augmented by having the user take a 5–minute break away from computer operation in each hour.

    Additionally:

    Suitable desks
    Adjustable chairs
Proper lighting including elimination of glare sources

As a raid leader you can only control one of these things. If you are worried about the health of your raiders then take 5 minutes every hour. Additionally the average human is more capable of retaining focus on a single task if they take this break.

Current guild raids: 3 nights a week, 3 hour raids. Normally do around 2 hours then a 5 minute break, then 1 more hour.

Previous guild I was in was 4 nights per week, 4 hour raids. We did 2 hours then a 15 minute break, then 1 hour and 45 mins.

I've joined guild pugs before where they did a 5 min break every hour which was a bit extreme for me.

This except I guess it's closer to 10 minutes.
