Do the Ancient Heirloom Armor Casing and the Ancient Heirloom Scabbard require that the toon applying the effect be level 100? Put another way, can a level 90 toon upgrade a lvl 60 heirloom to max to level 90?
Also, are the upgrades from 90-100 worth it or does Garrisons or questing provide better stuff? To what ilvl is it equivalent?

Nah, I did it on my level 85 toon (that is, applied the upgrade portion). Upgrading for 90-100 is for the speed, not the stats. The bonus XP is what you would be after.

from 90 to 100 you should equip all of those that provide experience boosts. for any other slot, especially weapon, you could equip a crafted one, with ilvl 630+ for maximum fun while leveling
