From what i've read, you pretty much equip it for demon bosses, ignore it for everything else. Don't have one tho, so take it with a grain of salt

I'll add that this is true when comparing vs heroic HFC trinkets. When comparing against mythic trinkets, the heirloom will generally always lose, except for some fringe cases such as Mythic Manno or Xhul, where you can refresh the proc alot.
