I think it was Pokemon Stadium on the N64 as a kid. Raged so much one time that I removed the N64 stick with my teeth. Hopefully the game didn't really used it I think.
Capsized. Between the awkward controls and annoying enemies, the game frustrated me.
Southpark the shit of truth. Not so much the game, but the fucking steam country restrictions which meant that I can't play the god damn game I bought in the UK, not even as a lesser license. Fuck that shit.
CS:GO, filled with russian hackers and laggers that can't be bothered to use the MM tool to play in their own regions. + the Valve servers suck, especially after each patch they release..
Dark Souls has had it's moment, but only in PvP (again lag... (again no regions) the fuck developers?)

Mario Kart until I had to git gud. Then once I learned a bunch of creative shortcuts the game became boring because I'd always win.

Demon Souls - I threw that disc across the room swearing to never play that shit ever again so many times. But I kept trying again because I wanted to finish it just because I didn't want the game to win. I didn't even enjoy the damn thing.
Overwatch - The amount of ragequits after having like 3-4 bad games in a row. Ugh.
Anything with competitive play, RNG and lag.
I get pissed off at Overwatch sometimes because my mouse has a bad habit of disconnecting when I ult or when barely winning a 1v1. Actually just anytime at all, availability heuristic just makes me say otherwise.
I remember the last level of Psychonauts giving me quite a bit of grief. Luckily when I get really pissed at a game I just quit and stop playing it for half a year, so nothing broke. Unfortunately on PC that means I force quit non-online games (I don't rage quit on people) because I don't want the screen to stay up for more than a couple seconds, so I hope nothing software wise breaks/gets corrupted :P
I haven't played that game since the Xbox 360 came out and that just made me wince. I remember hating those guys
Edit: If I get tilted in OW, I just quit the game for the day after the match ends. For instance, had a horrible game today where I had 0% hook accuracy after 8 or so hooks -- which just snowballed into me picking heroes badly and feeding... Yay!

Fallout 2. I had such a lame ass time playing it because I'd always die very god damn easily. I ended up throwing my laptop off the bed and shattered the screen once - because I got so mad.
Pugsly's Scavenger Hunt on the SNES - already a decent enough challenge on the easiest settings. I think I was close to needing therapy when I attempted it on the lowest life settings and with the 1-hit kill option.
Nightmare Busters on the SNES, despite its name, still gave me nightmares trying to beat it on the hardest setting. I dont think anyone told the devs this was suppose to be a game for the Super Nintendo and not an arcade game trying to reap maximum profit.
Mass Effect 3, no contest. The constant "thats an image from google...and thats a photoshop of an image from google, and talis face is a model from google painted purple" and so on. I wanted to finish the story but fuck me what a piss poor production that game is. The adoration it got from people really proved some folks have absolutely no standards anymore.
Medal of Honor: Frontline on Hard. I was all up for getting the platinum but halfway through I was like, "Fuck this shit."
fallout 4, for making me kill allies to finish the game...i have 380 hours played and havent finished the game. Also current WoW because of the dev's shitting on the community so much that it feels like a single player game with a chat box.
