Just completed Kara for the first time, and nobody in the group got credit for the quests involving killing Watcher Viz'aduum.

I had two quests:

-The Tower of Power
-In The Eye Of The Beholder

that both required me to kill him, and even after doing so, neither quest was marked as completed.

Is this happening to anyone else?
