Casketface was fucking hardcore. I miss that little bugger.

casketface means sarggesicht in german, which sounds almost the same as sackgesicht which in turn translates back into dickhead

some others i remember from levelling were Ratmuncher (if you're british you will laugh your tits off at this one, rat being a slang word for a vagina ) and Maggotrobber haha

weird, im from England and ive never heard of rat being a word used in that purpose...
