I'd post mine as I'm leveling up some more characters for fun. (incoming 30 level 85s!) but I'm only using the default with the unit-frames moved. No addons at all. I'm such a traitor.

Ones the target of target, the very top one is the focus frame and it's target. Little confusing but. Either way.

Looks very much like Forte and yes it can (on the bar where you see Mind Blast CD)

Whats the separate rage bar addon? And does it work for mana/focus/other power bars?

The debuffs are Fortexorcist and the ragebar is an addon called Ragebar.

I'm curious why you'd need a separate rage bar when there's already a power bar associated with the player unitframe.

If you didn't understand shit of what I just said, there is a sticky from Treeston.

for imgur, you do what was said above but you change the actual img tag url to http://i.imgur.com/KlkZP(m).jpg, adding a S for small, M for medium or L for large. So it'd appear like this:
