Problems with SoV/SoCor

I've noticed on several occasions that I lose the 5 stack of the debuff even though I was attacking the boss, facing him the whole time. This has happened most regularly on Putricide but happened again today on Archavon my logs show that I am hitting the boss with melee swings but for some reason the stacks drop.

I was wondering if anyone else was getting this happen and how they fixed it or if they are just dealing with it.

Archavon can easily be explained if you happen to go into the cloud for long enough.
