For DPS there should be a passive ability or talent that allows your physical attacks to damage the targets armor, or make an exposed wound on them. This should allow your diseases to tick faster for their duration or refresh themselves on the current/original target or just jump to other targets with the same debuff.

So what you want is for your auto attacks to proc sunder armour on the target while also increasing tick speed of diseases, refreshing diseases and spreading them automatically. Somehow I can't see this happening.

It would not be a sunder armor effect, the point of the armor thing is that you make an exposed wound or some sort of cut on the target allowing your diseases to be more effective.
