Bugged character?

Hello guys! I need some advice:

Yesterday when i was doing Putricide i noticed that many of my abilities were missing when i used them (Judgement, CS, even Vindication) which is odd since I have 286 hit rating and that never happened. Lately I have been doing BG's and running ICC, so i swap gear and talents a lot, so I tried to switch to my pvp set and then back to my pve gear and apparently it solved the problem, but later when we fought against Sindragosa it happened again.

So my question is: is my character bugged in any way? And if it is, how can I fix it? I use Mik's Scrolling Battle Text to track my abilities.

IIRC Putricide has a debuff that reduces chance to hit.

I am pretty sure i was avoiding the bombs unless that I didn't noticed it (which could happen). But why then it happened again against Sindragosa?

Putricide i agree with other person - you're blind. Give me a log to prove your misses on Sindragosa then i'll believe you, til then you're making it up in a defensive manner.

Don't believe you, no reason for it on sindragosa unless you are below 8%/7% with hit aura.

IIRC Putricide has a debuff that reduces chance to hit.

To add to this, on Heroic Mode, the is 100% miss chance (instead of 75%).
