I'm very interested in the original plan of a "Tier 1 epic, Tier 2 legendary, Tier 3 artifact" system. Has Blizzard confirmed that this was their original plan? Where can I read more about it?

The original game files included red and gold item text, though no items of these qualities were included. The gold item-name text was later associated with heirloom items.

The original itemization rules are attached to leveling items from 1-60. You'll see that the ilvl is connected to the level at which a character can equip the item.

Additionally, the traditional 13 ilvl tier-up (which was standard until hard mode itemization messed it up) is
almost mathematically identical to an item quality tier-up holding ilvl equal.

    Compare these:

    Savage (blue ilvl 200) http://www.wowhead.com/item=40778
    Hateful (purple ilvl 200) http://www.wowhead.com/item=40783
    Deadly (purple ilvl 213) http://www.wowhead.com/item=40786

It would probably be a good idea of each raiding "tier" had its own associated color. For example:

    Tier 1: Purple
    Tier 2: Bronze
    Tier 3: Silver
    Tier 4: Platinum
    Tier 5: Onyx

Or any easily distinguishable color from gray/white/green/blue. This would be for each expansion, so Kara would of been Purple, Sunwell would be Onyx, then Nax would be purple again, ext. PvP gear would be the same, but have different colors.

But honestly, it seems waaaay to late in WoW's life cycle to really mess with any of this.

First of all, this is very confusing. When there are three colors, it's pretty reasonable to expect people to know what they mean. When there are eight or nine different colors, it gets difficult to remember whether teal is better than periwinkle, or whatever.

And second of all, as I keep trying to explain, item quality is not just the color of the name, it's also a variable in determining the stats to be allocated. The difference between a blue and a purple of the same ilvl is mathematical. If you're moving two variables, then the relationship between two items of different tiers becomes harder to quantify.
