Prediction for shattering

4.0.3 contains the new world, but it will still be the same for us playing until 4.0.3a switches the new world on. That's how I've come to understand it

Yes, 4.0.3 lays everything down, puts everything in including the new world, but the new world will not happen until they flip the on switch with 4.0.3a

They should gimme at least 2 weeks so i can get a new raceclass combo to 80

4.0.3 is currently in the PTR and Blizz said 4.0.3a will also be tested before Cataclysm. So, before the PTR ends I wouldn't expect them to happen.

Why would you think you could have the 'new world' without the expansion installed?

The changes to the old world is going to happen for everyone not just those who buy the cataclysm xpack. This was mentioned I know more than once by the blue posters in regards to this.

The world is changing for everyone, the new areas and content will be exclusively for those who buy the box HOWEVER the changes to Azeroth and the updated quests, gnome starting area, new troll area will be available to all no matter what box you have.
