Yes, you have to log all the way out and then back in before any new content will be available to you.

You do have to log out completely. Make sure you close out of WoW totally.

No, I think you skipped the part where you can't log for 120 tries, because the servers are overwhelmed.

As mentioned in the following thread, you will need to log out to the login screen. (the one with deathwing sitting on stormwind) If you are playing when the switch happens, you will recieve a message telling you to do this.
