Apparently, it's a world drop. Just luck you got it in a dungeon.

So you are saying there is loot in WoW that is not added in an addon ??? This is an outrage!!
Everybody report ingame so the GM's can fix and remove this item!!

It is indeed a world drop. There are around 5-6 others as well, and none of them show up on atlas from what I can see.
