It was explained in the episode worprz.
as far as i know it was only speculated, i think it was briefly mentioned once an old episode, but nothing really set in stone. and some sources say 13 is the limit set by the timelords, so it no longer applies anyway.

if you look here it seems to be that it was stated there was 13incarnations, but it also may have just been a rule set by the time lords, and the rules may be broken, especially since they are not around anymore.
Plus who knows what River giving The Doctor all of her Regenerations did to the process as well.

I wonder if I am the only one who wishes they weren't afraid to delve more into the doctors past. I mean would it really be that bad to lessen the air of mystery around the doctor?
