was duelling a shammy outside oggy. fight was coming to and end. i brought the fight near one of the pigs. hoping he would hit it somehow . and just as i did my last swing to win the duel.. the pig attacked and killed him. oh the lolz from everyone hehe :P

My "stupid death" winner is jumping off a cliff/floating object only after which I realise that I hit my land mount keybind rather than the one for my flying. In at least one instance, I was on the mount for several seconds before the plunge to my doom, ample time to have noticed that my protodrake was looking suspiciously like a talbuk.

I think auto flying on an epic flying mount. Then, being how my destination was for, I alt-tabbed and read MMO-champion. I forgot what I was doing. I tabbed back to WoW...and I died from fatigue. Best/Worst. Death. Ever.

Shadowstep on magmaw first thing. Dead. next attempt: Same thing. for 5 times. After the fifth, he was removed from raid, and his guild note said "do NOT let in raid"

i say watching my guildies try to be tricky and jump off the elevator in bwd and fail :P once had one slow fall into nefarians place and die from lava, and sometimes you cant help gaining agro because it may be the tanks fault as well

Mage in some BWD pug i was in slow falled off the elevator right into Nefarian's Lava pit.

I've seen worse (and it was my fault):
Our Druid Tank comes in after a corpse run. I'm already standing on the elevator. I Lifegrip him to me, so he'll be faster. Just as I release the button, the elevator starts going down. Because of the momentum from Leap of Faith and the fact that he's going to where the elevator was, he falls down. I try to save the situation by Levitating him. He goes straight for the lava pool in the middle. For a moment it looks like he's going to make it across, but he ends up in the lava anyway.
