I understand your question, and the answer is most likely a technical one, and much better posed on the official forums.
Personally I didnt know that was the behavior, but if it is you might want to open a ticket about it as a bug. Seems like it would be unintended if there was nothing that you could gain from going into what is effectively an empty instance.
I was a victim of the same problem in Molten Core this week :<. Didn't know it applied across the board though.
Asked a GM about it and they can't un-do boss kill saves, but he was happy to forward the issue to someone else. Meaning there is no rationale for it happening and that they'll probably fix it sooner or later. So it should probably be able to get to Blizzard's attention with a post on the official forums.
Oh yes I didn't know either. I had cleared completely a Nax and then a guild mate wanted to solo it and when he got in with me in the group he was completely saved without doing anything like killing a boss. I do not know if you have to click 'accept' or anything.
But it's known by many people; another guild mate knew for a long time.

I made a post in official forums of EU but I don't trust it to be read to be honest. Especially EU doesn't have much traffic of blue.
