It's definetly Thrall's fault... if by Thrall you mean "Metzen".
The fault is horrific bad writing on Metzen's part. Pure and simple.
Led the Horde to victory in Northrend.
Brought the Goblins into the Horde, thus providing the faction with Oil - something they have lacked in large quantities since the Second War (Thrall just recommended them)
Brought the Dragonmaw back to the Horde (Ignoring the ham-handed "turn 'em evil" that has occured recently. The entry of the Dragonmaw was a great move forward on the Orcs side of things)
Brought the Taunka into the Horde.
Furthered Horde interests on Kalimdor including expansion into valuable lumber resources.
Devised and executed a brilliant Military victory over Theramore. (You may not like it, but it was)
Kept discipline within the Orc ranks of the Horde through an adherence to the Orc Warriors code of Honor.
And then he went nuts, but those are things he did to clearly show his growing potential.
Garrosh a success in Northrend? They slaughtered alliance soldiers for no reason when they were fighting at one of the massive scourge gates in Icecrown, destroying the offensive against the scourge and giving them plenty of corpses to raise.
Garrosh showed himself to be a complete psycho with baseless hatred of humanity right from mid-wrath. This really is Thrall's fault.

What if thrall always was evil and that was his plan ? Genoicide of alliance race's and purging of weak, and then using garrosh as victim to hide his own crimes ?
What if this so called "Go'el" is real evil mastermind of azeroth ? And we are playing right into his hands ?
Honoring best friends is not a good way to lead a band of militaristic Orcs who longed to see battle and the glory days of the Horde. Hell, Thrall himself acknowledged the Orcs were not fond of his leadership style, that they wanted a leader that would bring them glory in battle like Garrosh. Basically, Thrall gave the Horde what it should have never had, otherwise it would turn out like the Old Horde. Which is precisely what happened.
Besides, Thrall put Saurfang as Garrosh's advisor for a reason and he was fully aware of Garrosh's dickish tendencies for a long time, what with the challenge just before the Scourge arrived in Orgrimmar and Garrosh's inability to step down from any sort of challenge even when the fate of the world is at stake in the pre-Ulduar cinematic.
So yeah, Thrall done goofed pretty bad there.

This has been known for a long time... but you're painting him darker than he really is. If you read the novels you realize that Thrall appointed Garrosh because he was the popular choice amongst the people and trusted that responsibility would temper him. He didn't appoint Garrosh because of a desire to become more powerful or more renowned, he appointed Garrosh in an act of desperation, as he couldn't be both a Warchief and a Shaman, with the coming of the Cataclysm.
He did not. In 5.2 Thrall is rather shocked at what the Kor'kron is doing to the Darkspear. Plus, it wouldn't be in his character.
Garrosh was pretty happy when Thrall gave him that power... and no, not everyone knew Garrosh would be a bad Warchief. Those crowds wouldn't have cheered so hard if they didn't think he was a good choice.
