"Lover" is a pretty generic term, as is "paramour". "Mistress" carries a certain nuance.

People pay for stuff they can get for free all the time. They also pay more to get something at a higher quality or more to their liking. The adage about milk and cows applies here: Why buy a cow and bother with keeping it fed, etc, when you can simply buy the milk. Some people are farmers, others just like milk.
