It's Lord Waycrest, like the others have said and he's the big dude in Drustvar. Except he's kinda... not. Anymore. Anyway, he's probably the Drustvar Dungeon Boss.
It's never specified, afaik. The Lich King did not raise every Death Knight personally; Razuvious was the one picking through the corpses with a handful of necromancers in Acherus. Even were it the case that all worgen Death Knights were raised by the Lich King personally, that only implies that they are more resistant to, not immune to, undeath.
Worgen are resistant to the Plague of Undeath as they are all normal and magical diseases, but their corpses can still be raised into undeath and their wills bent by Necromancers of suitable skill (of which the Lich King would certainly qualify). Being slain by one of the Scourge or anything else afflicted by the Plague of Undeath isn't going to spread to the Worgen - their natural resistance trumps that kind of passive conversion. But acted on directly they're still as vulnerable as any other race when it comes to undeath, and insofar as we know there is no mortal race in the Warcraft universe that is patently immune to the state of undeath itself.

It's during the Silverpine quest, the citizens on Fenris Isle actual want to become Worgen to escape the Forsaken curse.
Also Blizzard explicitly stated that the reason Worgen can become DK is BECAUSE of Arthas/Lich King being so powerful in Necromancy.
Why are humans who drink the blood of worgen unable to be raised as Forsaken?
Not only are the Val'kyr less powerful than the Lich King when it comes to raising the undead, but the worgen curse also makes raising them into undeath far more difficult than it is for normal humans. The worgen curse has roots in both the Emerald Dream (through the wolf Ancient, Goldrinn) and the holy power of the goddess Elune. In addition, those worgen who imbibe the waters of Tal'doren—through the ritual they undergo to maintain balance between the worgen curse and their humanity—have a further resistance to the corruption of undeath.
