Mainly MM+ around 13-15 and heroic raiding

That’s not how tanking works. You need to balance survivability with dps. Good try being edgelord though.

My answer would be:

Blood Spatter Scale - it's awesome and basically has no drawbacks
Shard - almost as good as BSS

Gland is a risk/reward, and you're doing +15 and heroic, you don't need to be optimizing like that. Slimy organ is stamina (ew) and a trickle of healing or a burst heal that takes a few seconds to work, so to me it doesn't really help you stay alive and its got no dps component.

Nope, simming only maximise dps. No balance here.

Too many variables to account for - how good are your healers? How aggressive do you need to be on dps to beat the fight? Are there huge damage spikes that an on-use trinket would counter? How often are you dying?

Slimy Consumptive Organ - Dogshit, just a health boost that is basically worthless.
Blodd-Spattered Scale - Literally BiS for every tank. Must have, even.
Shard of Annhylde's Aegis - Less dogshit than the plaguefall one, but wouldn't call it good.
Titanic Occular Gland - I mean, not the worst but for a tank it's really not needed.

Keep the scale, always, forever, never change it.
Equip the Organ as a secondary (if you find yourselfs dying or close to dying much), else use the Aegis for some extra damage.

Notable replacements for the secondary trinket would be:
So'leah's Secret Technique
Ticking Sack of Terror
Inscrutable Quantum Device
Overcharged Anima Battery

If you play mythic plus at all, change to one of these if you can.

For raiding, it literally doesn't matter (keep the Scale tho at all times).

Don't give out braindead advices please.

Simming is for pure dps. Which is not the goal here, that's why you don't sim tanks or healers (unless you are some cheeky logger or smth).

But Scale value drops significantly if there's only one target? I would rather go with the cheat death trinket for raiding (Weave of Warped Fates) or something like Reactive Defense Matrix.

So like, Taragrue and Guardian?
Just because it's a raid it doesn't mean there is only 1 target. And even then, it's a chunky shield. If you need a cheat death you're tanking wrong.
