Or
Iran - 6

USA - 2

Goal difference takes priority to head-to-head. Technically Iran could draw with USA in the last giving this scenario:
Wales - 4
Iran - 4
England - 4
USA - 3

This would require Wales beating England with 6 goals, while Iran - USA draws with at least 3-3 or Wales beating England with 7, so I think this is the least likely scenario(unless I missed one) ;P
