It happens once in awhile for me too, seems like beta bug testing was successful.

Do you mean on boss fights or any death?
I've only ever seen it reset on a death in a boss fight and tbh thats the way I thought it should be working, if you die out in the world or on mobs in an instance it doesnt reset it

similar experience for me, also doomguard refuses to switch targets which is annoying.
