You can only get gear from each boss once per week, can run it as many times as you like though

Elder Charms can only be used on a kill on which you're eligible for loo.t (i.e your first kill of the week)

I would like to see a blue of this. Towelliee stream last night he killed Feng LFR for a 2nd time, wasn't eligible for loot but still had option to use Coin AFAIK.

I ran vault 3 times and it asked me to roll every single time I killed the boss

Same with sha of anger, killed it once on my monk, forgot to loot claw, killed it again, rolled again.
