It basically means that it's more important than anything else you could use those resources on, and that you want to focus your resources on maximizing its uptime.

For example, you have 100 RP and Timmy is in Bruce Banner mode. You could summon Gary the Gargoyle for 60 RP, or you could dump Death Coils to get Dark Trans up sooner and put Timmy in exciting Hulk Mode™! Spend the RP on Timmy.

You have an Unholy Rune and 5 stacks of Shadow Infusion. You could Scourge Strike. Or you could Hulk out Timmy. Spend the Rune on Timmy.

Try to imagine your Ghoul not as a pet, but as a buff on you that says "Increases all damage you cause by a stupid amount". It's your version of Inquisition for Ret. You want to keep that buff up as much as possible.

It's top prio right after diseases and soul reaper I believe. Always try to brute force a DT by pooling runic power. Especially since they made the DT buff 100% again it's very important to keep high uptime on this. On cleaveheavy fights I believe it could even be a priority over soul reaper.

    How to cheese DT:
Step 1: Get a reliable timer for its duration.
Step 2: Get too 100 RP when its at 3s or less but still on.
Step 3: Sit on a Sudden Doom Proc.
Step 4: Spam that shit the instant Timmy runs out of juice.
Bonus: If you can AMS for good numbers right at the end too, you win The Golden Cheese. You also get three Internetz if you stood in fire to do it.
You now just spent ~5 gcd outside DT before putting it back up.

I like to call this common sense rather than cheesing.

    As for:

Haven't looked at Unholy lately, but I'm pretty sure sitting on a Sudden Doom for more than a GCD or two at max is a DPS loss.

He means hold the proc when SDoom pops right before DT falls off. It's easier if you have a swing timer going so you can play Proc Chicken better.
