Well, that's the Rogue elements to it. Permanent death, completely random item and monster placement, etc. Part of being good at Roguelikes is learning to downplay the bad luck and play up the good luck. You can easily beat the game with 0 items or power-ups, it just takes practice and patience.

So... can you beat it with 1 heart, 1 damage, 1 range, 1 tear. No item, no space item, no trinket, all the way to polaroid ???? baby thingy.

It's usually done with Isaac, since he's the vanilla character with no abilities. Everyone else starts with something you can't get rid of - extra damage, extra speed, extra health, Whore of Babylon, etc. Well, it assumes you never use the D6, of course.

And people have done it. There are no rooms where you are guaranteed to take damage. I've seen examples of all of these rooms, and every single one of them gives you a way out.
